Continuing in July, registration for athletes in 2018/2019 will be online with Family ID at www.familyid.com

CMS Annual Sports Physicals

YOU MUST HAVE A PHYSICAL EVERY YEAR TO PARTICIPATE IN MIDDLE SCHOOL ATHLETICS.

Please note the following in regards to paperwork deadlines:

Paperwork deadline:

  • Fall Orientation - paperwork is due on the day of your 1st practice
  • Winter Season - paperwork is due on the day of your 1st practice
  • Spring Season - paperwork is due on the day of your 1st practice

Paperwork includes the following:

  • Physical form
  • "I Will" Contract

Eligibility:

Students who have ANY grades lower than a P (PP, SBP, IWS) and/or low effort & conduct scores are eligible on a probationary status.

Probation means that they must bring up their academic, effort, and conduct grades to all passing by the end of the quarter. That

allows for a half a quarter to turn things around before they lose eligibility. For students on probation, the new policy outlines

what the adults are expected to do to support their students.

  • Athletic Director will communicate with each students' teachers;
  • Athletic Director will meet with students on probation;
  • Athletic Director will coordinate academic and behavioral interventions for students and teachers;
  • Athletic Director will maintain student probationary records.

Parking & Transportation:

Parking is allowed at CMS. There is NO PARKING allowed in front of dumpsters. Please keep all fire and emergency lanes clear.

Transportation:

  • Student athletes must ride the bus to and from all competitions;
  • Student athletes must have parents sign a release form to be able to ride home with parents after a competition;
  • Teams may stop to eat when at an away game competition unless they are 50 miles away (one way).

Baseline Testing:

  • A diagnostic assessment tool to better manage concussion injuries;
  • Conducted in the Stevens High School Library, under direction of SHS Athletic Trainer as it is computer-based
  • Conducted each year to update information
